When free radicals outnumber antioxidants, it can lead to a state called oxidative stress. Which if left unchecked can lead to DNA damage and even death in some people.
Things that can cause a over oxidative effect on the body are:

Alcohol intake
Toxins
High blood sugar levels
High intake in polyunsaturated fats

To much intake of iron and magnesium
To much or to little of oxygen to the body
